Lincoln Street staff strive to make every aspect of your life less complicated, including the often challenging and complex social service system. Our Service Coordinators are experts at connecting individuals, their families and guardians with the right staff and area resources to meet their needs.
Whether you are new to Vermont’s developmental services, new to LSI services or a long term recipient of our services, Service Coordinators provide only the services you need and want, recognizing the value of ongoing dialogue between the individuals we serve and other team members so that services can fluctuate as needs change.
Service Coordination is based upon the following cornerstones of our agency’s culture:
- Effective advocacy that respects each individual and family’s culture.
- Commitment to individual planning processes that drive clinically and fiscally sound services.
- Facilitating a strong team that is respectful, accountable, and open to new ideas and approaches.
- Providing Individual Service Agreements, which is a contract for services, that are personalized and outcome driven.
“The Review Team saw knowledgeable, dedicated case managers who have a real understanding of who each of the individuals on their case loads are and the best ways to support them and the people providing the services to them.”
