Navigating the service system doesn't have to be unnecessarily, challenging or complex with assistance from LSI Service Coordinators. Whether you are new to Vermont's developmental services, new to LSI services or a long term recipient of our services, LSI's Service Coordinators are here to assist you when you need it.
The focus of Service Coordination is based upon the following cornerstones of our agency's culture:
- Effective advocacy that respects each individual and family's culture.
- Commitment to individual planning processes that drive clinically and fiscally sound services.
- Facilitating a strong team that is respectful, accountable, and open to new ideas and approaches.
- Providing Individual Service Agreements, which is a contract for services, that are individualized and outcome driven.
LSI Service Coordinators lead the team that surrounds each individual and family served according to the cornerstones of our culture. They supervise, support and monitor the quality of services provided by LSI home providers, community supports or employment support staff that serve the individuals on their caseloads. This enables them to effectively and flexibly adjust services according to an individuals needs and applicable regulations.
