One of the most important choices you make is where to live. At Lincoln Street we provide supports for living with your natural family or for living with a non-related home provider.

Family Supports:

Supports for individuals who live with their own families is a niche that Lincoln Street is proud of. Our Family Services Team focus heavily on partnering with each family to find the right balance of support. The key to our high rates of satisfaction is our ability to listen and to respect each individual's lifestyle and family's values. We collaborate in negotiating effective services that leave the individual and family in control of the service decisions that our teams implement.

Home Providers:

Our home providers serve a wide variety of individuals, from those with challenging medical needs, communication challenges and for those individuals who desire to build independent skills to move into their own living environment with less extensive supports.

Many of the individuals we serve have been living with the same home provider for over 15 years and have developed nurturing and enduring relationships. We are proud of the skill and patience our Shared Living Program Team take in assisting individuals and families to make the right match. They are especially meticulous in seeking home providers who share Lincoln Street's philosophies related to human dignity and the team approach in which an individual being serviced has an equal voice in developing his or her program. Thorough background checks and extensive training are all part of the process.

Respite Networks:

Respite is an important time for caregivers to recharge and for individuals receiving service to build additional friendships. A key component of our successful support of families and home providers is our emphasis on supporting them to hire safe accessible respite care resources.

Families and home providers often struggle with advertising for respite, interviewing applicants, and having applicants come to their home. Our Service Coordinators are available to support families and home providers in locating safe and accessible respite care resources.
